An annual evaluation report is helpful to both funders and grantees. It is a tool that enables us to reflect and grow from the program by examining lessons that have been learned during the year, the impact (short-term and long-term) on the population served, and what changes might be beneficial in similar programs in the future. The extent of annual program evaluation reports are often determined by the overall scale of the program. Recognizing the time investment involved in preparing judicious reports, we ask that reports be no longer than five pages. The report should provide a summary of the program’s activities, challenges, successes, next-steps and an accounting of all expenditures. All grant recipients must prepare an Annual Evaluation Report. It is due on the one-year anniversary of your project start date. The standard project start date is no longer than 30 days after the receipt of awarded funds, unless otherwise agreed upon.
